The tax clearance form, and all incentive application forms are available to download, save, fill out electronically, and print, so you can complete them at your convenience and e-mail, fax or mail them with the required documentation. (Acrobat Reader 5.0 is required to use these forms.)
Steps:

1.

Select a form.
Please select the desired form(s) from the list on the right by clicking on them.

Complete and print the form.
When you select a form, one of two things will happen.
- The form will be displayed in your browser window OR
- The browser will ask you to save the form on your desktop as a file. Once you save the file you can open it in Adobe Acrobat Reader.
After you open the form, you can fill it out on the screen and print it immediately.

Sign and date the form.
Make sure you sign the form before you fax or mail it.

Submit the form to the Commercial/Industrial Market Manager.
Fax, e-mail or mail the completed form, plus any supporting documentation, to the appropriate number or address on the second page of the form.
