The tax clearance form, and all incentive application forms are available to download, save, fill out electronically, and print, so you can complete them at your convenience and e-mail, fax or mail them with the required documentation. (Acrobat Reader 5.0 is required to use these forms.)

NOTE: Electric Unitary HVAC, Gas Heating and Variable Frequency Drives applications are under development for proposed 2012 changes. Please check this web page for updates.

Steps:

1. Select a form.
   Please select the desired form(s) from the list on the right by clicking on them.
2. Complete and print the form.
   When you select a form, one of two things will happen.
   • The form will be displayed in your browser window OR
   • The browser will ask you to save the form on your desktop as a file. Once you save the file you can open it in Adobe Acrobat Reader.
   • After you open the form, you can fill it out on the screen and print it immediately.
3. Sign and date the form.
   Make sure you sign the form before you fax or mail it.
4. Submit the form.
   Fax, e-mail or mail the completed form, plus any supporting documentation, to the appropriate number or address on the second page of the form. Our mailing address is:

5. For the Custom Measures Spreadsheet:  You must enable macros to use this spreadsheet. For those using the Microsoft Office 2007 version of Excel, you may need to install the latest update to enable macros to run. About Adobe Acrobat Reader, Approval, and
Adobe PDF files: With free Adobe Acrobat Reader software you can view, fill out, and print Adobe PDF files across a broad range of hardware and operating systems. You can also save blank eForms on your computer. (Acrobat Reader 5.0 or higher is required to use these eforms.)

To save information you have typed onto an eForm, Adobe Acrobat Approval 5 or above is required; this program can be purchased directly from Adobe.
