The project registration form, the tax clearance form, and all incentive application forms are available to download, save, and print, so you can complete them at your convenience and fax or mail them with the required documentation. (Acrobat Reader 5.0 is required to use these forms.)
Steps:

1.	Select a form.
	Please select the desired form(s) by clicking on the list at the right.
2.	Complete and print the form.
	When you select a form, one of two things will happen. The form will be displayed in your browser window OR The browser will ask you to save the form on your desktop as a file. Once you save the file you can open it in Adobe Acrobat Reader. Print and complete the forms at your convenience.
3.	Sign and date the form.
	Make sure you sign the form before you fax or mail it.
4.	Submit the form to the Commercial/Industrial Market Manager.
